If you live in our practice catchment area and would like to register with us, please complete a registration and health check form.
For your convenience you may like to download, print, complete and then simply send the forms to us by post.

Alternatively, you may like to come to the Health Centre and ask a receptionist for the necessary forms to complete or seek advice.
You can also telephone 01454 412599 and ask reception to post the forms to you.
If you are a new patient to the area and need an appointment we will ask you to complete the forms prior to seeing the doctor.
On the form, you will be able to say which practitioner you would prefer to see. However, you will be registering with the practice rather than an individual GP.
You will be asked to provide proof of living in the area i.e. utility bill etc.
